The Launch of the ‘Heavenly’ Health + Healing Hubs in Fort Worth’s 76104 and 76105 Communities
“We are elated to announce a transformative milestone: a generous award from Texas Health Resources has enabled Healthier Texas to collaborate with Pastor Kyev Tatum, Sr., Teresa Wagner, DrPH, CPH, RDN/LD, CPPS, FNAP, CHWI, DipACLM, CHWC, and the Ministers Justice Coalition of Texas to pilot the ‘Heavenly’ Health + Healing Hubs in Fort Worth’s faith-based communities.
This initiative is of utmost importance for the 76104 and 76105 ZIP codes, two of the most underserved areas in Fort Worth. These communities experience some of the highest rates of chronic illnesses—such as diabetes, hypertension, and heart disease—in Texas. Limited access to healthcare, widespread food insecurity, and systemic economic challenges have contributed to alarming health disparities. In 76104, the average life expectancy is just 67 years, the lowest in the entire state of Texas.
The ‘Heavenly’ Health + Healing Hubs aim to address these disparities head-on by establishing vital spaces within faith-based settings, where trusted leaders and organizations can deliver accessible, culturally attuned health interventions. These hubs will focus on four key pillars:
• Preventative care to reduce the prevalence of chronic diseases.
• Nutrition education to combat food insecurity and encourage healthier living.
• Mental health services to support emotional well-being and resilience.
• Chronic disease management to enhance quality of life and improve long-term health outcomes.
By engaging faith leaders and leveraging their trusted voices in these communities, the hubs will deliver solutions that resonate deeply and foster meaningful change. These spaces will also empower residents with the tools, education, and resources needed to take control of their health and cultivate lasting well-being for themselves and their families.
